How We Connect You to Services
Your Intake Appointment
Please bring your health card, proof of Indigenous lineage (e.g., status card, Métis/Inuit registration, letter from your community) and a list of your medications to your intake appointment. If you have already started filling out the intake form, please bring a copy with you.
You should expect the intake appointment to last up to one hour, depending on your needs. We use this time to get to know you a bit more and address any wellness concerns you may have. You can also learn about all of the SOAHAC services that are available to you and your family.

- Are you a parent/guardian wanting your child (under 18) to become a new patient or client? Please note you will need to fill out a separate form for each child. Use this form: SOAHAC Intake for additional children (PDF).
